Pregnancy with 4T risk factors too young, too old, too short birth spacing, and too many children contributes significantly to maternal and infant complications. Pregnancies occurring at ages <20 or >35 years, with intervals of less than two years, and parity of ≥4 are associated with increased risks such as anemia, hypertension, hemorrhage, preterm birth, low birth weight, and even maternal and neonatal mortality. Data from the Ciruas Community Health Center in June 2026 reported 190 cases of 4T pregnancies, with the highest concentration in Cigelam Village, totaling 14 pregnant women. This community service program aimed to enhance the knowledge of pregnant women with 4T risk regarding potential complications, the importance of the Birth Planning and Complication Prevention Program (P4K), and awareness of family planning for pregnancy regulation. The intervention combined “CERIA 4T” health education with a local food innovation, mung bean milk pudding (PUKIS CERIA 4T), to support maternal nutritional needs. The program was conducted on July 16, 2026, in Cigelam Village, involving 14 pregnant women. It employed face-to-face Communication, Information, and Education (KIE) methods, including interactive discussions and leaflet distribution. Participants also received training on preparing PUKIS CERIA 4T as a nutritious snack, along with product samples. The implementation consisted of preparation, execution, and evaluation stages using pre-test and post-test assessments. The results demonstrated an improvement in knowledge scores from an average of 7 to 9.5. This indicates that the “CERIA 4T” education and local food innovation effectively improved understanding, awareness, and nutritional independence among high-risk pregnant women.
