ANALISIS HUBUNGAN KELENGKAPAN PENGISIAN RESUME MEDIS PASIEN RAWAT INAP DENGAN PENENTUAN KODE PENYEBAB DASAR KEMATIAN DI RUMAH SAKIT UMUM DHARMA YADNYA DENPASAR

ABSTRACT
Kode penyebab dasar kematian adalah kode yang di tetapkan sebagai sebab dasar terjadinya kematian, dalam menentukan ketepatan kode dibutuhkan kelengkapan informasi berupa resume medis. Hasil observasi awal menunjukkan Ketidaklengkapan Pengisian resume medis sekitar 70 (14%) dari 511 rekam medis (resume medis) dan ketidaktepatan penentuan kode penyebab dasar kematian yang dimana terdapat kode ”R” sebagai penyebab dasar kematian. Tujuan penelitian ini adalah mengetahui hubungan kelengkapan pengisian resume medis pasien rawat inap dengan penentuan kode penyebab dasar kematian di Rumah Sakit Umum Dharma Yadnya Denpasar. Rancangan penelitian ini adalah deskriptif analitik kuantitatif, dengan jenis penelitian kohort retrospektif. Jumlah sampel dalam penelitian ini yaitu 25 resume medis. Instrumen penelitian berupa ceklist dengan tehnik pengumpulan data observasi dan dokumentasi. Hasil penelitian menunjukkan bahwa kelengkapan pengisian resume medis pasien rawat inap pada bagian diagnosis (p value = 0.001) ada hubungan signifikan sedangkan bagian indikasi pasien dirawat ( p value = 0.821), pelayanan penunjang ( p value = 0.571) pemeriksaan fisik (p value = 0.561) tidak ada hubungan signifikan dengan penentuan kode penyebab dasar kematian. Kesimpulan dalam penelitian ini adalah kelengkapan pengisian diagnosis resume medis berperan penting terhadap ketepatan penentuan kode penyebab dasar kematian di Rumah Sakit Umum Dharma Yadnya Denpasar.

ABSTRACT
The underlying cause of death code is a code that is determined as the underlying cause of death, in determining the accuracy of the code required for completeness of information in the form of medical resumes. Preliminary observations show that incomplete medical resume filling is around 70 (14%) of 511 medical records ( medical resumes) and inaccuracy in determining the underlying cause code that contains the “R” code as the underlying cause of death. The purpose of this study was to study the relationship between the completeness of filling patient medical resumes at the Dharma Yadnya General Hospital Denpasar. The design of this study is descriptive quantitative analytic, with a retrospective cohort study type. The number of samples in this study were 25 medical resumes. The research instrument consisted of a checklist with observation and documentation data collection techniques. The results showed that the completeness of filling in the inpatient medical resume at the diagnosis (p value = 0,001) had a significant relationship while the indicative part of the patient was treated (p = 0.821), supporting services(p value = 0,571) physical examination (p value = 0,561) there is no significant relationship with determining the underlying cause of death code. The conclusion in this study is the completness of filling medical diagnose resumes play an important role in the accuracy of determining the underlying cause of death code at the Dharma Yadnya General Hospital in Denpasar
