ANALISIS HUBUNGAN KETIDAKLENGKAPAN REKAM MEDIS PASIEN RAWAT INAP DENGAN INSIDEN KESELAMATAN PASIEN DI RSU SURYA HUSADHA UBUNG

Abstract

ABSTRAK
Patient safety adalah suatu sistem pelayanan kesehatan yang menjamin pasien agar
terhindar dari cedera yang tidak diinginkan dan kejadian nyaris terjadi yang dapat merugikan
pasien. Salah satu sasaran keselamatan pasien yaitu ketepatan identifikasi pasien. Ketepatan
identifikasi pasien dapat diukur menggunakan rekam medis dengan dua identitas. Identitas pasien
minimal harus tercantum dalam rekam medis adalah nama dan nomor rekam medis pasien.
Berdasarkan studi pendahuluan di RSU Surya Husadha Ubung, 34 (68%) dari 50 rekam medis
tidak lengkap dibagian review identitas pasien. Penelitian ini bertujuan untuk mengetahui
hubungan ketidaklengkapan rekam medis pasien rawat inap dengan insiden keselamatan pasien di
RSU Surya Husadha Ubung tahun 2018. Rancangan penelitian menggunakan deskriptif analitik
kuantitatif dengan jenis penelitian model kohort. Metode pengumpulan data menggunakan
observasi yaitu checklist. Analisis data yang digunakan adalah uji univariat dan uji bivariate dengan
alat ukur statistic chi-square. Hasil penelitian menunjukkan tidak terdapat hubungan
ketidaklengkapan nama pasien pada rekam medis rawat inap dengan insiden berkaitan dengan
ketepatan identifikasi pasien di RSU Surya Husadha Ubung tahun 2018 dengan p-value 0,361 >
0,05 dan tidak terdapat hubungan ketidaklengkapan nomor rekam medis pasien pada rekam medis
rawat inap dengan insiden berkaitan dengan ketepatan identifikasi pasien di RSU Surya Husadha
Ubung tahun 2018 dengan p-value 0,361 > 0,05.

ABSTRACT
Patient safety is a health care system that guarantees patients to avoid unwanted injuries and near
misses that can harm patients. One of the goals of patient safety is the accuracy of patient
identification. The accuracy of patient identification can be measured using medical records with
two identities. The minimum patient identity must be listed in the medical record is the name and
number of the patient's medical record. Based on preliminary studies at Surya Husadha Ubung
General Hospital, 34 (68%) of the 50 medical records were incomplete in the patient's identity
review section. This study aims to determine the relationship of incomplete medical records of
inpatients with patient safety incidents at Surya Husadha Ubung Hospital in 2018. The study design
uses quantitative descriptive analytic research with cohort model type. The method of collecting
data using observation is a checklist. Data analysis used was univariate and bivariate tests with
chi-square statistical measuring devices.The results showed there was no correlation between the
incomplete name of the patient in the inpatient medical record with the incident related to the
accuracy of the identification of patients in Surya Husadha Ubung Hospital in 2018 with p-value
0.361> 0.05 and there was no relationship between the incomplete medical record number of the
patient in the inpatient medical record. hospitalization with incidents related to the accuracy of
